Download Here for a copy of our LSP Membership Application and follow our easy to use directions! Or, if you e-mail or call us, Commonwealth will send you our LSP Member Application. In any case, just complete it and return it to Commonwealth together with the payment as directed. Commonwealth will send your Membership materials and Membership card setting forth your Membership start date and account number.
As a new Member, following your first full month of membership, you will become eligible to receive up to 5 hours of legal service (the “Member’s Allocation”) during the 12 month Cycle commencing on your Effective Date. Commonwealth’s Providers have a list of some common legal services for its Members. Each legal service has a number of hours associated with it. When you have a legal issue and need to talk to an attorney, call our toll-free number (1-877-305-6500) (it's also printed on your membership card for your convenience) and leave a message for Legal Services and your call will be promptly returned. That’s it!
